Disparities Information for Patients
There are racial and ethnic disparities in pneumococcal vaccination rates among African American, Hispanic, American Indian, and Alaska Native populations.  In addition, these populations typically experience more of the chronic health conditions that put them at risk for pneumococcal disease.  Below you will find suggested template language that can be incorporated into newsletters, e-mails, or other educational materials for your patients in these populations.  The language can be tailored to address specific risks and disparities in your community.
